It has been one of the greatest honors of my professional life to walk alongside each of you on your healing and wellness journeys. As your provider, I have witnessed your strength, resilience, and growth—and I am deeply grateful for the trust you’ve placed in me and our practice.
With heartfelt reflection and care, I am writing to share a difficult decision. Despite the rapid growth of our practice—driven by your support and belief in our mission—we have faced a number of unexpected challenges as a new, single-provider clinic. After much consideration and collaboration with our partners, we have made the very difficult decision to close the practice effective October 31, 2025.
This decision was not made lightly, and I recognize the frustration, sadness, and disruption it may cause. Please know that our priority remains your well-being. We are committed to making this transition as smooth as possible by:
• Offering appointments through the closing date
• Providing appropriate referrals to ensure continuity of care
• Ensuring you receive 90 days of medication to allow time to establish with a new primary care provider
For those of you who are also patients of Awaken Alternatives, that practice will continue to offer Virtual Care Self-Pay visits and services as appropriate.

No Show Policy
At Markovitz Medical, we strive to provide the best possible care for all our patients. To ensure that our schedule remains efficient and allows us to accommodate the needs of all our patients, we have implemented the following No Show Policy:
Cancellation/Rescheduling Policy for Insurance-Based Appointments:
Patients must cancel or reschedule their insurance-based appointments at least 24 hours in advance.
Failure to provide at least 24 hours' notice will result in a no-show fee of $50. This fee is not covered by insurance and is the patient's responsibility. Will automatically be charged to Credit/ Debit Card on file.
After 3 no-show visits, the patient will be dismissed from practice.
After 2 No show visit patient may be asked to only make same day appointments until consistency in appointment arrival is proven.
Cash-Paid Services:
All cash-paid services must be paid in full at the time of booking.
There are no refunds for cash-paid services. Not attending the appointment forfeits payment and session. If special circumstances for cancellation arise you pay place a request in writing to have your payment applied to an account credit at MarkovitzMedicalLLC@gmail.com. Markovitz Medical reserves the right to deny this request.
General Policy:
Patients are encouraged to arrive 15 minutes prior to their appointments. If a patient is more than 15 minutes late, it may be considered a no-show, and the no-show fee or full charge for cash-paid services will apply.
By adhering to this policy, we can ensure that our schedule runs smoothly and that all patients have the opportunity to receive the care they need. We appreciate your understanding and cooperation. By scheduling an appointment with our clinic, you acknowledge and agree to the terms outlined in our No-Show and Cancellation Policy.
